Diabetes Care, Education & Monthly Support Group

Diabetes Care and Education:

Diabetes care and education is the process of helping someone understand and manage diabetes so they can stay as healthy as possible and feel confident in their daily care. Our Weeks Medical Center Diabetes Educator can help individualize your diabetes plan.

Diabetes Education Includes:

Blood Sugar Management
Medications-oral, Insulin or Injectables
Nutrition
Physical Activity
Diabetes Technology
Emotional Support
Family Education
Preventing Complication

To schedule an appointment with our diabetes educator, please reach out to your primary care provider for a referral to WMC Diabetes education.

Diabetes Care and Education Monthly Support Group:

Diabetes Support Group meets every 4th Tuesday of the month from 7pm-8pm, at Weeks Medical Center in the Hospital Board Room.

Join us in a welcoming, confidential space space, where people living with diabetes can come together and share their journey; getting the extra support you need.

Our group provides education, tools and peer support to help you feel more confident managing diabetes day to day.

Each month will offer new topics with guest speakers or round table discussions.

No registration is required.
