WMC Launches Chronic Care Management Program

Lancaster, NH – August 28, 2026 – Weeks Medical Center (WMC) is launching a Chronic Care Management (CCM) program designed to support people living with two or more chronic health conditions.

Chronic Care Management (CCM) is a proven, patient-centered model that helps individuals with multiple chronic conditions better manage their health through check-ins, care coordination, and support between office visits. Many insurance plans, including Medicare, cover CCM services. Benefits of the CCM program include developing a personalized care plan, medication management, and monthly check-ins by phone with a member of your clinical care team.

“We know how hard it can be to manage multiple health conditions, and we want to help make it easier,” said Monique Hand, Director of Population Health at North Country Healthcare. “Maybe people have questions about which medications to take and when, or whether new or worsening symptoms warrant a doctor’s visit. Through a monthly phone call, we can check in with our patients and make sure that their questions are answered and that they are doing alright. This way, we can address concerns as they come up, and patients don’t have to wait for their next office appointment to talk to someone about their care.”

WMC Primary Care patients that have two or more chronic conditions, such as diabetes, heart disease, high blood pressure, arthritis, or COPD, may be eligible.
