Androscoggin Valley Hospital, Upper Connecticut Valley Hospital, Weeks Medical Center, North Country Home Health & Hospice Agency, and Coös County Family Health Services are pleased to announce that they are conducting a collaborative Community Health Needs Assessment (CHNA) to evaluate the overall health needs of the area. The CHNA provides many organizational and community benefits like identifying community health needs, health disparities, and key service delivery gaps.
We are asking community members to participate in a survey that will help us complete this assessment. The survey will run until January 31st and your anonymous response will help us understand diverse community perspectives, prioritize the health needs in our area, and develop an action plan to address those needs.
